Discussion of enrollment, training, and updated background checks obtained and placed on file.

Comments:
This renewal inspection was initiated by licensing staff and concluded on 09/27/2022, between the hours of 10 am to 11:45 am. There were 5 children present (12 points), ranging from toddler to preschool-age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 5 children records, 1 staff record, and 2 household records were reviewed.

Information gathered during the inspection determined that there was (1) non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the home.

Violations:
Standard #: 8VAC20-800-90-C
Description: Based on review, 2 of 5 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for children (who were not exempt from the immunization requirements according to subsection B of this section) once every six months for those children under the age of two years.

Evidence -
1. On the date of inspection (09/27/2022) the immunization documentation on file at the FDH for Child #1 was dated - 11/23/2021, and the immunization documentation on file at the FDH for Child #2 was dated - 03/23/2022.
2. On the date of inspection, based on the information on file, both Child #1 and Child #2 were under the age of two years and held no immunization exemption of file.

Plan of Correction: Will obtain this documentation from the parents of Child #1 and Child #2.
