Discussion on staff records and license requirements as a family day home provider.
The Fairfax County Zoning Permit conditions list on page 2 must be primary residence.

Standard #: 22.1-289.036-B-4 Description: Based on staff records review, the family day home provider did not obtain copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #1 (date of employment 9/03/2013) central registry results expired on May 2022. A copy of central registry results are updated every 5 years.
2. Staff #2 (date of employment 2/18/2021) resided in the state of Maryland in the preceding five years and did not have child abuse and neglect registry or equivalent registry results available in the file available for review during the inspection.Plan of Correction: Staff #1 expired 8.2017 has gotten new form notarized-waiting on new form from CPS.
Staff #2 received MD form will be mailed by 10/10/22
Standard #: 8VAC20-800-30-A Description: The provider did not ensure compliance with these standards and the terms of the current license issued by the department and with relevant federal, state or local laws, and other relevant regulations.
Evidence:
1. Based on provider's statement the provider resides at the residence two nights a week.
2. Based on provider's statement the provider travels with an overnight bag from the family day home residence to another residence.
3. Based on provider's statement of residing at the facility 2 nights at week, this is not the primary's residence, Provider does not meet the definition of a license family day home because not provider's primary residence.
Provider is not in compliance with terms and conditions of license.Plan of Correction: I do consider this my primary residence. If instructed to stay/spend more nights is required by written law I will do as pan of correction. Please see note below.
Standard #: 8VAC20-800-150-D-1 Description: Based on observation, the provider did not current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care for assistant is left alone with children in care.
1. During a walk-through, Staff #3 was left alone with children on the playground while Staff #2 was assisting a child in the bathroom.
2. Staff #3 was left alone with children in care when came staff came inside from the playground to help children with water containers.Plan of Correction: Discussed with staff to never go inside other teacher is not present.
Standard #: 8VAC20-800-150-D-2 Description: Based on observation, the provider did not current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care for assistant is left alone with children in care.
