Comments:
An unannounced renewal inspection was conducted on October 3, 2022 from approximately 10:50 am to 12:30 pm. There were 2 children in attendance. 1 staff record, 2 household member records and 2 children?s records were reviewed. All areas of the home were observed and areas where the children are cared for were inspected.

Interviews were held with the provider throughout the inspection. The designated areas for childcare were deemed appropriate and are equipped with age appropriate toys to meet the needs of the children. Children were observed playing with homemade playdough, racing cars, coloring, and playing with a children?s bowling set while the licensing inspector was at the home.

Evacuation drills were conducted monthly. Shelter in place drill conducted two times with the most recent on 06/24/22.

The center?s first aid kit was inspected and found complete. The home had a working flashlight and battery operated radio.

There were violations cited as a result of this inspection.

Standard #: 8VAC20-800-140-B-3
Description: Based on review of records the provider failed to maintain current certification in CPR. Evidence: The record for the provider contained CPR certification that expired on 09/12/2022.

Plan of Correction: Per the provider "cpr certification done 10/12/2022."

Standard #: 8VAC20-800-140-B-4
Description: Based on review of records the provider failed to maintain current certification in First Aid. Evidence: The record for the provider contained First Aid certification that expired on 09/12/2022.

Plan of Correction: Per the provider "first aid certification done 10/12/2022."

Standard #: 8VAC20-800-150-D-1
Description: Based on review of records, the provider failed to ensure that an assistant has current certification in CPR. Evidence: The record for staff #2, an assistant who is left alone with children contained CPR certification that expired on 09/12/22.

Plan of Correction: Per the provider "CPR certificate is cuirrent."

Description: Based on review of records, the provider failed to ensure that an assistant has current certification in First Aid. Evidence: The record for staff #2, an assistant who is left alone with children, contained First Aid certification that expired on 09/12/2022.

Standard #: 8VAC20-800-180-A
Description: Based on a review of caregiver records and interview the provider did not obtain results of a follow-up tuberculosis screening at least every two years from the first screening. Evidence: The record for caregiver #1 contained documentation of a tuberculosis screening dated 04/24/2018.

Plan of Correction: Per the provider "TB placement test done 10/03/22. Reading 10/6/22. Negative."

Standard #: 8VAC20-800-320-B
Description: Based on observation, the provider failed to ensure that child-resistant protective covers were installed on all unused electrical outlets and surge protectors accessible to children under five years of age. Evidence: There was a surge protector on the floor in the room where the children are cared for and there was an outlet without a cover in the room where the children are cared for.

Plan of Correction: Per the provider "child-resistant protective cover was installed in inspector presence. Surge protector is placed behind desk and is unreachable."

Standard #: 8VAC20-800-330-A
Description: Based on interview, the provider failed to have an operable and accessible landline telephone during the hours of operation. Evidence: The provider stated that she did not currently have an operable landline telephone.

Plan of Correction: Per the provider "landline telephone repaired 10/11/2022."

Standard #: 8VAC20-820-120-E-1
Description: Based on observation and interview, the provider failed to ensure that the most recently issued license and inspection finding were posted on the premises. Evidence: The license or most recent inspection were not visible. When asked, the provider stated that the license and most recent inspection were not posted.

Plan of Correction: Per the provider "the license and most recent inspection are posted in visible sight."
