Comments:
An announced Initial inspection was conducted on October 6, 2022. The inspector was on-site from approximately 11:15 am until 12:45 pm. The provider plans to provide care to children six months through twelve years, eleven months. A capacity of (9) nine children has been requested and is recommended based on child care space. Care will be provided in three rooms. The home is equipped with a variety of age appropriate equipment and supplies including materials for gross motor, art, literacy, and dramatic play. The provider will provide meals and allow parents to supplement meals due to allergies or dietary restrictions. A walk through of the entire home was conducted during today's inspection. Transportation will not be provide. The inspector reviewed compliance in the areas of administration, physical plant, staffing, emergencies and nutrition. A total of one caregiver record was reviewed. A conditional (6 month) license will be recommended. An unannounced follow-up will be conducted within 60 days to determine compliance with standards not determined at the initial inspection.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-800-760-A-1
Description: Based on inspection of the first aid kit, the provider did not ensure the kit contained all required items.

Evidence:

The first aid kit did not include triangular bandages as required

Plan of Correction: The applicant ordered the triangular bandages. Per the applicant: the bandages will arrive on Saturday.

Description: Based on interview, the applicant did not obtain a working portable battery-operated weather band radio, and extra batteries that shall be kept in a designated area and available to caregivers at all times.

Evidence:

The applicant stated she did not have a working portable battery-operated weather band radio.

Plan of Correction: The applicant ordered the radio. Per the applicant: the radio will arrive on Saturday.

Description: Based on observation, the applicant did not ensure all areas of the home, inside and outside, were maintained in a clean, safe and operable condition.

Evidence:

The wooden shelfing unit in the room to be used by children had a loose shelf on the bottom of the unit. The shelf was not attached on one end exposing two nails.

Plan of Correction: Per the applicant: I will have someone repair the shelf.

Standard #: 8VAC20-800-330-A
Description: Based on observation and interview, the applicant did not obtain a landline telephone, excluding a cordless or cell phone that is available, operable and accessible during the family day home?s hours of operation. An operable landline telephone is one that does not require electricity to operate.

Evidence:

A landline cordless phone was observed. The applicant stated she did not have a phone that did not require electricity to operate.

Plan of Correction: The applicant ordered a phone while the inspector was on site. Per the applicant: The phone will arrive on Saturday.
