Technical Assistance:
Provided consultation on standard:
1) 60 B - a child?s record shall contain the name, address and phone number of two designated persons to contact in case of an emergency
2) 90 A - before the child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized or provide an affidavit that the administration of immunizing agents conflicts with the parent?s or child?s religious tenets or practices
3) 830 A - the emergency evacuation procedures shall be practiced monthly
4) 830 B - shelter-in-place procedures shall be practiced a minimum of twice per year

Comments:
An unannounced monitoring inspection was conducted from 12:45 - 2:00 pm with the provider and her assistant. There were four daycare and two household children present, ranging from nine months to four years of age, for a total of 18 points. Children were observed eating lunch and napping. Lunch served today: pasta and ground beef with zucchini and tomato sauce or corn, rice, and steak for those with special dietary needs, along with pears and milk. Four child records, two caregiver records, the emergency drill log, daily attendance, and menu were reviewed. The provider is not currently certified in Medication Administration Training (MAT) and cannot administer medication to children in care. Standard #: 8VAC20-800-170-A
Description: Based on review of documentation, and an interview with the provider, not all caregiver's had, at the time of hire and prior to coming into contact with children, a current TB test/screening. Evidence: the record for caregiver #2, who was observed supervising children, did not contain documentation of a TB test/screening.

Plan of Correction: Caregiver #2 has an appointment to obtain a TB test screening .
