Technical Assistance:
New staff orientation model form will be emailed to the provider as a resource.

Annual training for all staff will be reviewed at the next licensing inspection.

Discussed preparation of food needs to occur in the basement to ensure appropriate staff supervision and to remain within the maximum points allowed if a second assistant is not present.

Comments:
A monitoring inspection was conducted today. A total of 9 children (21pts) with 2 staff were present. An adult visitor with a child was also present at the beginning of the inspection. The children were positioned on the floor was observed in an exersaucer. Observation of the middle floor of the home and the programming space located in the basement was made. The crib room, bathroom and food preparation area was also observed. No medication needs or identified food allergies at this time. A total of 5 children's records and all staff and household records were reviewed. Some violations found today. One repeated/uncorrected violation found. Standard #: 8VAC20-800-750-B
Description: Based on observation, one topical ointment was found accessible to children.
Evidence:
One topical ointment was found in the storage compartment which was hung on a door. The compartment with the ointment was accessible to children.

Plan of Correction: The topical ointment was disposed of. No topical ointments will be stored in this area unless it is out of reach to children.

Description: Based on observation, a table was not properly cleaned.
Evidence:
The table used for eating and activities was not washed with soap and water or detergent solution and rinsed with water.

Plan of Correction: We will use warm soapy water to wash the tables down and water to rinse the table after cleaning.

Standard #: 8VAC20-800-200-A
Description: Based on staff record review, staff training has not been completed.
Evidence:
Staff #3 was hired on 1/6/2022. Orientation training which is required within the first week of employment has not been completed. This is repeated and uncorrected from the last inspection.

Plan of Correction: This staff will have the orientation training and the documented training will be sent to the Licensing Inspector for review.

Description: Based on observation of the food preparation area, a small appliance not in use was plugged into the outlet.
Evidence:
A toaster which was not currently in use and located on the counter in the food preparation area was found plugged in.

Plan of Correction: The toaster was unplugged and will remain unplugged unless being used.

Description: Based on observation, the diapering surface located in the bathroom is no longer absorbent.
Evidence:
The diaper mat used for diapering has cracks in the surface and is no longer absorbent.

Plan of Correction: We will no longer use this surface and will use the second surface located in the crib room. We will purchase a new pad for the changing table in the bathroom

Standard #: 8VAC20-800-690-E
Description: Based on observation, the receptacle used for diaper disposal requires a caregiver's hand to touch the exterior surface to dispose of a soiled diaper.
Evidence:
The foot operated receptacle no longer lifts hands free when disposing a diaper.

Plan of Correction: A new step on pale will be purchased.
