An unannounced inspection was conducted on December 13, 2022, from 7:50am to approximately 9:15am. There were five children present totaling 11 points and the provider. The Inspector reviewed four children?s records and provider?s record. The Inspectors observed staff and children?s interactions throughout the inspection. Children were observed during free play, storytime, breakfast, and handwashing. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented. The violation is listed on the violation notice issued to the center and were reviewed with the provider at the exit interview.

Standard #: 8VAC20-800-100-A Description: Based on record review and interview, it was determined that the provider did not ensure that they obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. The record for Child #1 (date of enrollment 28/11/2022) did not have documentation of a physical examination by or under the direction of a physician prior to attendance or within 30 days after the first day of attendance.
2. Staff #1 confirmed that there was no documentation of a physical examination for Child #1 in their record.Plan of Correction: The plan of correction: Staff #1 will obtain the physical examination for Child #1and place the documentation in the record.
