Technical Assistance:
1. Discussed requirements for each child to have a complete enrollment record even when they are siblings.
2. Each child is to have a completed enrollment record by the first day of attendance.
3. Reviewed process for reporting paid sick days for VDSS subsidy program.

Comments:
An unannounced monitoring inspection was conducted at 2:20pm. Observed 1 child = 2 points with the Provider. During the inspection a teenage child arrived to the home and there were multiple household members present. Ratios and supervision were in compliance. The child was playing with toys, watching TV, reciting colors and numbers, and coloring pictures. The provider was engaged with the child and activities. The daycare areas of the home were found to be clean and sufficiently supplied with toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Standard #: 8VAC20-800-100-A
Description: Based on records reviewed, it was determined that 3 of 5 children?s records did not include a physical exam within the first 30-days of attendance.

Evidence:

1. Child #3 ? First date of attendance was 5/18/22, and to-date no physical exam on-file.
2. Child #4 ? First date of attendance was 9/18/19, and to-date no physical exam on-file.
3. Child #5 ? First date of attendance was 9/1/22, and to-date no physical exam on-file.

Plan of Correction: The provider said that she would request physical exams for all children and ensure they are filed in each record as required.

Standard #: 8VAC20-800-60-B
Description: Based on records reviewed, it was determined that 3 of 5 children?s records did not include required information. Evidence: The records for Child #3, #4, and #5 did not include medical insurance number.

Plan of Correction: The provider said that she would request the medical insurance information and write it on the form.
