Comments:
An unannounced monitoring inspection was conducted today between 8:55am and 10:30am. There were 8 children present with 2 caregivers for a total of 21 points within the supervision guidelines. The children were observed playing with various manipulatives, working on puzzles, dramatic play and drawing. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were reviewed during this inspection. There were no medications on site at this time. The provider completed a total of 16 hours of training during the last licensing period. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 8VAC20-800-750-A
Description: Based on observation, the provider failed to ensure that when topical skin products such as diaper ointments are used, the products shall be labeled with the child's name. .
Evidence:
1. Balmex ointment was not labeled with a child's name.

Plan of Correction: I will write the name on the tube now.
Violation corrected during inspection.

Standard #: 8VAC20-800-750-B
Description: Based on observation, the provider failed to keep topical diaper ointment inaccessible to children.
Evidence: Three topical diaper ointments (Balmex, A & D, and Vaseline) was stored in an open container that was sitting on the floor.

Plan of Correction: I will move the ointment so it is out of reach.
Violation corrected during the inspection.

Standard #: 8VAC20-800-90-C
Description: Based on record review and interview, the family day home did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1) Child #6 (DOB 7/23/20) most recent documented immunization record is dated 11/15/21.
2) Child #8 (DOB 6/1/22) most recent documented immunization record is dated 6/1/22.
3) The provider acknowledged additional immunization record had not been obtained for child #6 and child #8.

Plan of Correction: I will email the parents today and tell them I need an updated immunization record.

Standard #: 8VAC20-800-210-A
Description: Based on record review and interview, the family day home failed to have caregivers complete 16 hours of annual training.
Evidence:
1. Caregiver #2 (DOH 12/24/2020) record did not have documentation of additional annual training.
2. The Provider acknowledge that caregiver #2 did not complete the minimum of 16 hours of additional training.

Plan of Correction: I will ensure the caregiver completes the annual training this year and have her complete at least 10 hours of training by June 9, 2023.
