Comments:
An unannounced renewal inspection was conducted on January 18, 2023. There were 6 children present (18 points) with 3 caregiver's supervising. The home, children's records, caregiver records, household member records, evacuation logs, attendance logs, first aid kits, cribs and menu's were reviewed. The children were observed playing with floor toys and preparing to go outside. Areas on non-compliance were cited on the Violation Notice. Standard #: 8VAC20-770-60-C-2
Description: Based on a review of caregiver records, the provider failed to ensure that the licensed family day home shall have a central registry finding within 30 days of employment.

Evidence:
Caregiver #3's record did not have documentation of a central registry finding within 30 days of employment. Employment date: 02/21/2022.

Plan of Correction: I will re-send CPS paperwork today and will contact inspector as soon as it is returned to me.

Standard #: 8VAC20-800-780-B
Description: Based on a review of children's records, the provider failed to annually review with the parent the emergency contact information required in 8VAC20-800-60-B to ensure the information is correct and to obtain the parent's signed acknowledgement of the review.

Evidence:
Child #1's last review was 01/10/2022.
Child #4's last review was 10/01/2021.

Plan of Correction: I will catch up on completing annual reviews and keep updated in the future.

Standard #: 8VAC20-800-60-B
Description: Based on a review of children's records, the provider failed to obtain all required documentation for each child enrolled.

Evidence:
Child #3, #4 and #5's records don't contain documentation of the name, address, and telephone number of each custodial parent's place of employment.
Child #5's record does not contain documentation of the names of persons other than the custodial parents who are authorized to pick up the child.

Plan of Correction: I will get the information from the parents and put in each child's file.

Standard #: 8VAC20-800-180-A
Description: Based on a review of caregiver and household member records, the provider failed to obtain fore each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening.

Evidence:
Caregiver #1, #2 and Household Member #1's records do not contain documentation of a current Tuberculosis Screening. All three were tested 12/15/2020.

Plan of Correction: We will all have TB test completed within five days.

Standard #: 8VAC20-800-200-A
Description: Based on a review of caregiver records, the provider failed to ensure that substitute providers and assistant providers shall have orientation by the end of their first week of assuming job responsibilities.

Evidence:
Caregiver #3's record did not have documentation of orientation.

Plan of Correction: I will complete and document caregiver orientation.
