Comments:
An unannounced renewal inspection was initiated and concluded on 1/23/2023. The inspector was on site from approximately 9:28 am-11:03 am. There were 9 children in care, ranging in age from 6 months to 2 years, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, care of children, emergencies, caregiver training, preventing the spread of disease and nutrition. A total of 5 child records and 4 caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-800-570-A
Description: Based on observation and interview, the provider did not ensure that a caregiver did not exceed 16 points by using the point system to determine if an additional caregiver was
needed.

Evidence:
1. On 1/23/2023 at approximately 9:38 am, nine (9) children ranging in age from six (6) months to two (2) years were observed in the care of one (1) caregiver totaling 27 points. The caregiver reported that the other two (2) caregivers had left to take the school aged children to school. The two (2) additional caregivers arrived back to the home at approximately 9:43 am.
2. The provider acknowledged that the one (1) caregiver had over 16 points for approximately 43 minutes. She acknowledged that the one (1) caregiver had 27 points.

Plan of Correction: An assistant was hired starting 1/30/23 to ensure there is coverage in case someone is out sick. If there are not enough staff, children will be informed not to come so we do not exceed our capacity.
