Technical Assistance:
Provided consultation on standard:
1) 60: a child?s record shall contain the name, address and phone number for 2 emergency contacts
2) 80: within 7 business days of the child's first day of attendance, the provider shall obtain proof of the child?s age and identity (by viewing the birth certificate, birth registration card, notification of birth, or passport
3) 100: the provider shall obtain documentation of a physical prior to the child's first attendance or within 30 days after the first day of attendance
4) 120: the provider shall maintain a record for each caregiver
5) 560: monitoring each sleeping infant in one of the following ways:
a. by placing each infant to sleep in a location where the infant is within sight and hearing of a caregiver,
b. by in-person observation of each sleeping infant at least once every 15 minutes, or
c. by using a baby monitor
6) 200: the provider shall orient the substitute and assistant provider by the end of their first week of assuming job responsibilities
7) 790: the following telephone numbers shall be posted in a visible area close to the telephone: the responsible person for emergency backup care and the regional poison control center

Comments:
An announced initial inspection was conducted from 10:00 am - 12:00 noon with the provider and her assistant. There were four daycare children and one household child in care, ranging from eight months to seven years of age, for a total of 12 points. Children were observed participating in math time, eating a snack, listening to a story, playing outside and playing inside with age-appropriate toys. Snack served today: apple slices and graham crackers. Four child records and four caregiver/household member records were reviewed along with the daily attendance, emergency preparedness plan and menu. Childcare areas inside and outside were observed. The day care area is organized and well equipped with age/stage appropriate supplies, activities, and equipment for the ages of children requested. The first aid kit, battery operated flashlight and weather band radio were observed and in compliance. Two cribs were observed and meet Consumer Product Safety Commission (CPSC) federal safety standards for cribs. The provider has current certification in CPR and First Aid but is not current in Medication Administration Training (MAT) so cannot administer medications to children in care. Violations:
Standard #: 8VAC20-800-940-A
Description: Based on observation, the temperatures were not maintained at or below 40 degrees F in the refrigerator and at or below 0 degrees F in the freezer. Evidence: the refrigerator's temperature measured 44 degrees F and the freezer temperature measured 10 degrees F.

Plan of Correction: Will lower the temperatures in the fridge and freezer so that they are maintained at the proper temperatures.

Standard #: 8VAC20-800-320-A
Description: Based on observation, and interview, the provider failed to ensure that all small appliances were unplugged when not in use. Evidence: a toaster and hotpot in the kitchen were observed plugged in while not in use.

Plan of Correction: Appliances were unplugged during the inspection.

Standard #: 8VAC20-800-330-A
Description: Based on observation, and interview, the provider failed to ensure there is a landline telephone, excluding a cordless or cell phone, that is available and accessible during the family day homes' hours of operation. An operable landline telephone is one that does not require electricity to operate. Evidence: a landline telephone was not observed, and the provider stated that she did not have one.

Plan of Correction: Will obtain a landline telephone.

Standard #: 8VAC20-800-680-A
Description: Based on observation, not all caregivers washed their hands with liquid soap and warm, running water before preparing and serving food. Evidence: a caregiver did not wash their hands before preparing and serving the children their snack.

Plan of Correction: Retrain caregivers about handwashing requirements.

Standard #: 8VAC20-800-680-B
Description: Based on observation, caregivers did not ensure children?s hands were washed with liquid soap and warm, running water before eating. Evidence: children did not wash their hands before eating snack.

Plan of Correction: Going forward will ensure children's hands are washed before meals.
