Technical Assistance:
Technical assistance was offered related: upcoming renewal; background checks, TB screenings and crib sheets.

Comments:
A monitoring inspection was conducted on 2/1/23 from 11:00am to 12:15 pm. There were 11 children (34 points) with 3 caregivers. The children were observed in free play and eating lunch that was brought from home. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, emergencies and nutrition. There were no children on site with allergies and/or medications. A total of 5 children's records, 3 caregiver's records and 1 adult household member's record was reviewed. Emergency drill were current.

Violations:
Standard #: 8VAC20-800-750-A
Description: Based on staff interview, the provider did not obtain written parent authorization for a topical ointment used for a child in care.
Evidence: A diaper ointment was on site for Child # 4. The provider stated that she did not have written permission form the parents to apply this ointment.

Plan of Correction: Written authorizations will be obtained for all topical ointments.

Standard #: 8VAC20-800-960-E
Description: Based on observation, bottles were not labeled with children's full names and the date.
Evidence:
1. 5 infant bottles for children in care were not labeled with name and date.
2. 1 bottle for had a name but was not dated.

Plan of Correction: All bottles will be checked by all caregivers to ensure proper labeling.

Standard #: 8VAC20-800-100-A
Description: Based on records review, the provider did not obtain documentation of a physical examination for a child within 30 days of enrollment.
Evidence: Child # 3's first day of enrollment was 2/2/22. Child # 3's physical exam has not been obtained to this date.

Plan of Correction: The child is leaving. Moving forward I will ensure that the physical is obtained on time.

Standard #: 8VAC20-800-60-B
Description: Based on records review, children's records did not have complete information.
Evidence:
1. Child # 1and Child # 2's record was missing information for people authorized to pick up in case of an emergency.
2. Child # 2's record was annually updated. The last update was completed on 12/31/21.
3. Child # 3's record was missing the medical insurance policy number and authorization for emergency medical care.
4. Child # 5's record was missing the authorization for emergency medical care.

Plan of Correction: All missing information will be collected from the parents and records will be updated.

Standard #: 8VAC20-800-180-A
Description: Based on record review, the provider did not obtain a subsequent tuberculosis (TB) screening for each caregiver, every two years.
Evidence: Caregiver # 1's most recent TB screening expired on 11/30/22.

Plan of Correction: The TB screening will be updated.

Standard #: 8VAC20-800-280
Description: Based on observation, cleaners were not stored away from food and inaccessible to children.
Evidence: The kitchen counter where children's food and milk bottles were stored had a bottle of bleach and water solution. The bottle was within reach of children.

Plan of Correction: The bottle was removed during the inspection. Moving forward all cleaning supplies will be stored away from food and made inaccessible to children.

Standard #: 8VAC20-800-530-D
Description: Based on observation, linens were not cleaned when soiled.
Evidence: A crib sheet in one crib was dirty with stains and food crumbs all over it while in the crib.

Plan of Correction: Crib sheets will be checked to make sure they are clean.
