Technical Assistance:
Discussion that the Policy for the Administration of Medication at the FDH forms were incorrectly completed. Since the Provider does not currently hold MAT certification, she is prohibited from administering both over the counter and prescription medication. Non-prescription topical skin products are allowed to be administered with written authorization from the child's parent (provided that this product is not prescribed or prevented from being administered based on the manufacture's instructions. Please complete new forms to reflect accurately the status to which the Provider is able to administer (as indicated above).

The staff information sheet was reviewed with the Provider in order to make the submitted renewal application complete.

Comments:
This renewal inspection was initiated by licensing staff and concluded on 02/03/2023, between the hours of 10 am to 11:45 am. There were 2 preschool-age children present with 1 relative child (5 points), with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 2 children records, 1 household record, and 1 staff record was reviewed.

Information gathered during the inspection determined that there were/was (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the center home.

Standard #: 8VAC20-800-830-B
Description: Based on review, 2 shelter-in-place drills were not conducted over the past year.

Evidence -
1. On the date of inspection (02/03/2023), the drill log for shelter-in-place was left blank.
2. Although the Family Day Home was closed during November and December of 2022, there was still enough time during the other 10 months of the year to have practiced these 2 drills with the children in care.

Plan of Correction: At least 2 shelter-in-place drills will be conducted prior to the end of this calendar year.

Description: Based on review, 2 of 2 children records did not include all of the required information and/or documentation.

Evidence -
On the date of inspection (02/03/2023), the following information and/or documentation was not available for review:
1. 1 of 2 emergency contact information for Child #1
2. 1 of 2 medical insurance policy number for Child #1
3. 2 of 2 Family Day Home Administration of Medication Policy form for Child #1, and Child #2
4. 2 of 2 Liability Declaration Form for Child #1, and Child #2.
