An unannounced renewal inspection was conducted on 02/07/2023. There was one 3 year old child, totaling 2 points, and one caregiver present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, care of children, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 4 children?s records, 1 caregiver record, 1 household member record and 3 pet records were reviewed. The inspector arrived for the inspection at 9:00 am and departed at 10:15 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-800-90-C Description: Based on record review, the provider failed to obtain documentation of additional immunizations for a child once every six months for children under the age of two years and once between each child?s fourth and sixth birthdays.
Evidence:
1. The most recent record of immunizations in the record for Child 1 was dated 12/02/2020. The record contained documentation to verify that Child 1 is 3 years old; additional immunization records were not obtained every six months while the child was under the age of two years. The record for Child 1 contained a documented start date of 07/22/2019.
2. The most recent record of immunizations in the record for Child 2 was dated 06/11/2018. The record contained documentation to verify that Child 2 is 7 years old; additional immunization records between the child?s fourth and sixth birthday was not obtained. The record for Child 2 contained a documented start date of 11/11/2015.
3. The most recent record of immunizations in the record for Child 3 was dated 03/29/2019. The record contained documentation to verify that Child 3 is 7 years old; additional immunization records between the child?s fourth and sixth birthday was not obtained. The record for Child 3 contained a documented start date of 11/11/2015.Plan of Correction: Will request the updated records from the parents.
