Comments:
The purpose of today?s visit was to conduct a renewal inspection during a routine unannounced visit approximately 11:20 am to 12:25 pm. Upon my arrival, I was greeted by the Assistant. Provider was off-site during the inspection. Licensing observed and documented the attendance. There were 12 children, totaling 24 points, present during today?s visit. The children were observed outdoor play, transition to handwashing and lunch. The children bring meals from home. Staff were engaged and interactive with the children. Staff provided assistance to children when needed during the inspection.

Limited licensing requirements were monitored and reviewed. The Inspector reviewed areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-920
Description: Based on observation and staff interview, when meals or snacks are provided by the family day home, the menu for the current one-week period was not dated and kept on file one week at the family day home.
Evidence:
1. Staff stated snack menu is posted on the refrigerator the day.
2.The snack menu posted did not have the date.

Plan of Correction: By the end of the week, type/print menu for Mondays.

Standard #: 8VAC20-800-180-A
Description: Based on staff records review, the provider did not obtain written documentation for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
1. Staff #1 Tuberculosis Screening expired on 10/03/2021.

Plan of Correction: Staff #1 will schedule appt for next week

Standard #: 8VAC20-800-230-A
Description: Based on staff records review, the provider did not maintain written documentation of each caregiver's applicable education and programmatic experience, applicable first aid and CPR certification, orientation, annual training, and applicable medication administration training.
Evidence:
1. Staff #1 completed 5 out 16 hours of annual training.
2. Staff #2 completed 12 out of 16 hours of annual training.
3. Staff 3 completed 6 out 16 hours of annual training.

Plan of Correction: Staff #1,2, &3 will choose course and completed by the end of May
