Comments:

This monitoring inspection was conducted by licensing staff to include documents review, interviews and observations of the program. The inspection was initiated on February 6, 2023 and concluded February 6, 2023. There were 2 children present, age 2 years old, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, personnel, physical health of caregivers, caregiver training, physical equipment/ environment, care of children, preventing spread of disease, emergencies and nutrition. A total of 5 child records, I provider and 1 assistant record was reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Description:	Based on observation and staff interview the licensee did not ensure when meals and snacks are provided by the family day home, the menu for the current one-week period shall be given to parents or posted or placed in an area accessible to parents. Evidence: Staff #1 confirmed that the current week menu had not been posted and had not been given to parents.

Description:	Based on record review and staff interview the licensee did not ensure documentation of the review of the child?s emergency contact information as required. . Evidence: The provider confirmed that the following records have not been updated / reviewed by parent /guardian annually as required. 1)Child #1 record was last signed by parent on 1-10-2022 2) Child #3 record was last signed by the parent on 9-4-2021 3) Child #5 record was last signed by the parent on 12-21-2021.
