Technical Assistance:
Discussed the need to vary activities to include hands on activities for children.

Will forward the model form for medication authorizations.

Comments:
An unannounced Monitoring Inspection was conducted today. A total of 6 children with 2 staff (15pts) were present today. The children were sitting on the carpet watching TV at the beginning of the inspection. They transitioned to lunch time after washing hands. Lunch today was pasts with meat sauce, steamed broccoli, green beans and carrots and turkey meatballs with milk or water to drink. The children ate while watching TV. After lunch the children prepared for nap time. Two rooms are used for napping with both cribs and cots in each. A total of 2 children's records and all staff records were reviewed. One identified food allergy which requires an emergency medication. This inspection began at approximately 11:15am and ended at 1:20pm. Standard #: 8VAC20-800-700-A
Description: Based on review of medications, one authorization was not obtained.
Evidence:
An authorization for a prescribed medication for Child #1 was not signed by a physician.

Plan of Correction: The parent will provide the physician with the correct forms to complete and sign.

Standard #: 8VAC20-800-180-A
Description: Based on staff record review, subsequent TB screenings were not completed.
Evidence:
Staff #1 and #2 did not have a subsequent TB screening every two years as required.

Plan of Correction: Both staff will obtain TB screenings by the end of this month.
