Technical Assistance:
Licensing inspector discussed with provider requirements for 8VAC20-800-530-F.

Comments:
A monitoring inspection was initiated and concluded on March 14, 2023. The licensing inspector was onsite from approximately 11:45am until 2:00pm. The provider and assistant were available throughout the inspection. There were four children present, ranging from seven months to four years of age (total of 12 points) with two staff supervising. The inspector reviewed compliance in the areas of administration, care of children, physical plant, physical equipment and environment, staffing and supervision, programming, special care and emergencies and nutrition. A total of five children records were reviewed. Two staff were interviewed during the inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Violations:
Standard #: 8VAC20-800-240-A
Description: Based on observation and interview, the provider failed to ensure areas and furnishings of the family day home were maintained in a clean, safe and operable condition.

Evidence:
1.A gas heater with a screen in front it was observed in the play area and located within three feet from combustible materials including, but not limited to wood, paper and plastics. The screen was shorter than the height of the heater making the heater and open flame accessible to children.
2. During the inspection, a 2 year old child was observed standing on top of an item and looking down over the screen, which allowed access to the gas heater and open flame.
3.The provider acknowledged during the inspection, that the screen located in front of the gas heater had been replaced and did not ensure a safe condition.
