Technical Assistance:
Discussed the requirements for Household Members background checks. Discussed the Sworn Disclosure form. The link to the current form will be sent by email.

Comments:
An unannounced renewal inspection was conducted today from 10:00am-11:20am with the provider on site. There was 1 child with 1 caregiver. A sample of 1 child's file and 10 caregivers/household member's files were reviewed. The child was engaged in the following: toy play, drinking a bottle, listening to the provider read a book, handwashing and a diaper change. Interactions between the child and caregiver were positive. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. The physical space, first aid kit, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice. Standard #: 8VAC20-800-170-A
Description: Based on review of records, the provider did not obtain from each adult household member a current report of Tuberculosis Screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form.
Evidence:
1. Household Member #6 (Moved in 3/06/2023) did not have a Tuberculosis screening form on file with the provider.

Plan of Correction: He will do by this weekend.

Standard #: 8VAC20-800-240-A
Description: Based on observation, furnishings of the family day home outside were not maintained in a safe and operable condition.
Evidence:
1. A children's table on the outside deck had a large crack in the seat.

Plan of Correction: I will throw away for recycle pick up.

Standard #: 8VAC20-800-280
Description: Based on observation, potentially poisonous substances, such as deodorizers were not stored away in areas inaccessible to children.
Evidence:
1. In the bathroom that children use, two wallflower deodorizers were in an unlocked drawer and were assessable to children..

Plan of Correction: I removed those out.

Standard #: 8VAC20-800-320-B
Description: Based on observation, Child-resistant protective covers were not installed on all unused electrical outlets accessible to children under five years of age.
Evidence:
1. One unused outlet in the play area was not covered with a protective outlet cover.

Plan of Correction: I put outlet cover back in.
