Comments:

The licensing inspector conducted an unannounced monitoring inspection on Thursday, March 9, 2023 to determine the provider's compliance with licensing standards. The inspector was on site from 9:30am to approximately 10:55am. During the inspection, there were a total of six children in care totaling 13 points in the direct care of one caregiver. The children were observed in the primary area of the family day home playing with toys and interacting with one another. The children were later observed coloring and playing with the provider. All areas used for child care were inspected. The family day home has age and stage appropriate materials and supplies. The provider was observed having positive interactions with the children in care. The required information was posted to include: the license, inspection notice, emergency numbers, daily schedule, menu, fire and shelter-in-place drills, and various parent information. The first aid kit and non-emergency supplies were inspected and found complete. One caregivers' record, five children's records, and pet vaccination records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #:	22.1-289.035-A
Description:	Based on a review of records and interview, the provider did not obtain the satisfactory results of the fingerprint-based national criminal background check every five years. Evidence: 1) The record of Caregiver #1 contained documentation of a fingerprint-based national criminal background check dated 03/06/2018. The repeat background check should have been obtained prior to 03/06/2023. 2) During interview, Caregiver #1 acknowledged the fingerprints were not completed every five years as required.
Plan of Correction:	New fingerprints will be obtained as soon as possible. The results will be submitted to the Licensing Inspector upon receipt.
Standard #:	8VAC20-800-180-A
Description:	Based on a review of records and interview, the provider did not obtain a repeat tuberculosis (TB) screening for one caregiver every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: 1) The most recent TB screening the record of Caregiver #1 was completed on 12/20/20. 2) During interview, Caregiver #1 acknowledged an updated TB screening has not been obtained. The updated TB should have been obtained no later than 12/20/22.
Plan of Correction:	A new TB screening/test will be obtained on 03/14/23.
