Comments:
A renewal inspection was conducted on 03/15/2023. There were 5 children present, totaling 14 points, with the provider supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 2 caregiver/household records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations were reviewed with the provider during the exit interview.

Violations:
Standard #: 8VAC20-800-100-A
Description: Based upon review of children's records, the provider has not ensured that they obtain documentation of a physical examination by or under the direction of a physician prior to a child's attending or within 30 days after the first day of attendance.
Evidence:
The record provided for child 1 documented the first date of attendance as 8/15/2022. The child's record did not include the results of a physical examination.

Plan of Correction: The provider responded with the following:
The parent will be contacted and results of a physical examination will be obtained. In the future, documentation of a physical exam will be obtained within 30 days of attending.

Standard #: 8VAC20-800-280
Description: Based upon observation, the provider has not ensured that potentially poisonous substances are stored in areas inaccessible to children.
Evidence:
there were cleaning fluids with warning labels stored in an unlocked cabinet under the sink in the bathroom used by the children.

Plan of Correction: The provider responded with the following:
The cleaning fluids were moved to a location inaccessible to the children during the inspection. If potentially hazardous substances are stored under the bathroom sink, a child proof latch will be installed. Otherwise all poisonous substances will be kept in areas inaccessible to the children.
