Technical Assistance:
An updated Information to Parents model form will be forwarded.
An Orientation Training form will be forwarded to use to document the required training.

Comments:
An unannounced Monitoring inspection was conducted today. A total of 10 children (23pts) with 2 staff and one adult visitor were present. The children were transitioning to outside play at the beginning of the inspection. Children were observed retrieving their shoes and either putting their shoes on independently or receiving assistance from staff. Handwashing procedures were observed when the children returned inside in preparation for lunch. All 10 children sat at one table for lunch. Lunch today was sandwiches, steamed broccoli and fresh oranges. Staff were observed encouraging the children to eat and assisting those who needed help with eating. No medication needs and no identified food allergies currently. Observation of the entire program space such as the 2 main rooms, the crib/diapering area, bathroom and meal preparation area was made. A total of 5 child records were reviewed. Both staff records were reviewed. The program space was found maintained and neat. This inspection occurred between 10:15am and 12:30. Description: Based on review of background checks, background checks were not completed every five years.
Evidence:
Staff #1's last fingerprint search result was dated 02/26/2018. Staff #2's last fingerprint search result was dated 02/27/2018.

Plan of Correction: Fingerprint searches will be completed. Fingerprint results will be forwarded to the Licensing Inspector for review.

Standard #: 8VAC20-800-760-A-1
Description: Based on review of the first aid kit, the kit was incomplete.
Evidence:
Antiseptic cleaner was not maintained in the provider's first aid kit.

Plan of Correction: Antiseptic cleaner will be purchased and maintained in the first aid kit.

Standard #: 8VAC20-800-60-B
Description: Based on child record review, one child record was incomplete.
Evidence:
Child #5's record did not include the date the child started care.

Plan of Correction: The start date for this child will be documented.

Standard #: 8VAC20-800-70-B
Description: Based on review of child records, the provider failed to obtain a parent's written acknowledgment.
Evidence:
Child #2's record did not include acknowledgment that parents were notified of the following:
1. Requirement for the parent to inform the family day home within 24
hours or the next business day after his child or any member of the
immediate household has developed any reportable communicable disease,
as defined by the State Board of Health, except for life- threatening
diseases, which must be reported immediately.

4. Requirement for paid caregivers to report suspected child abuse or neglect
according to ? 63.2-1509 of the Code of Virginia.

from the website, including compliance history that includes information after
July 1, 2003.

Plan of Correction: I will have this parent sign the acknowledgement for the information to parents.

Standard #: 8VAC20-800-90-C
Description: Based on review of children's records, subsequent immunizations were not obtained every six months for a child under the age of 2 years.
Evidence:
Child #2 is under the age of 2 years. The last documented immunizations were dated August 2022.

Plan of Correction: Updated immunizations will be obtained from the child's parents.

Standard #: 8VAC20-800-200-C
Description: Based on staff record review, documentation of orientation training was not maintained for one staff.
Evidence:
Documentation was not maintained for Staff #2 that staff #2 received orientation training.

Plan of Correction: Staff #2 will document that orientation training was completed.
