Comments:
An unannounced monitoring inspection was conducted from 12:40 - 1:40 pm with the provider. There were four daycare children and one household child in care, ranging from 11 months to seven years of age, for a total of 12 points. Children were observed reading books, washing hands preparing for lunch, eating lunch and preparing for naptime. Lunch served today: chicken with pasta, salad, blackberries and milk. Four child records were reviewed along with the daily attendance, emergency drill log, and menu. The provider does not have certification in Medication Administration Training (MAT) so cannot administer medications to children in care. Description: Based on review of documentation and an interview with the provider, it was determined that the emergency evacuation procedures were not practiced monthly, as required. Evidence: there was no documentation of an evacuation drill for January, February or March of 2023.

Description: Based on observation, the temperatures were not maintained at or below 40 degrees F in the refrigerator and at or below 0 degrees F in the freezer. Evidence: the refrigerator's temperature measured 42 degrees F and the freezer temperature measured 4 degrees F.

Description: Based on observation, not all cribs in use had tight fitting sheets. Evidence: two cribs had loose sheets and there was a child in one of them. Loose crib sheets are a suffocation hazard and increase the risk of sudden unexplained infant death.
