Technical Assistance:
Discussion of upcoming expiration of TB documentation in 2023.

Discussion of the remain hours needed in 2023 to complete the annual training requirement. So far the Provider has obtained a total of 13.5 hours of training in a child related field.

Comments:
This monitoring inspection was initiated by licensing staff and concluded on 03/28/2023, between the hours of 10 am to 11:30 am. There were 5 children present (14 points), ranging from infant to preschool-age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 5 children records, and 1 staff record, and 2 household records were reviewed.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Violations:
Standard #: 8VAC20-800-70-A
Description: Based on review, 1 of 5 children files indicated that the Family Day Home (FDH) Medication Administration - Decision to Administer form was not completed.

Evidence -
1. On the date of inspection (03/28/2023), it was determined that the parent of Child #4 did not sign this form.
2. The start date for Child #4 was listed as 10/24/2022.

Plan of Correction: The parent will be contacted to have this form signed.

Standard #: 8VAC20-800-430-A
Description: Based on observation, access to the stairs was not prevented by a mounted gate.

Evidence -
1. On the date of inspection (03/28/2023) it was observed that the FDH's mounted gate (at the bottom of the stairs) leading into the child care area was off its hinges and was being held up by a toy box.
2. At the time of inspection, there was one child (Child #5) under the age of 2 years who was not developmentally ready to climb or descend stairs.

Plan of Correction: The gate will be remounted to wall and secured in place.
