Comments:
A monitoring inspection was conducted on 03/27/2023 from 2:40 pm until 3:40 pm. There were eight children present, ranging in ages from two to eight years old, with one staff supervising for 12 points. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and onestaff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Description: Based on the review of five child records, the program failed to ensure a signed acknowledgment of an annual review of emergency contact information for five children.

Evidence:
1. The last date that a review was completed for Child #1 was 02/04/2019.
2. The last date that a review was completed for Child #2 was 03/15/2022.
3. The last date that a review was completed for Child #3 was 12/07/2019.
4. The last date that a review was completed for Child #4 was 12/12/2019..
