8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes

Comments:
A Subsidy Health and Safety monitoring inspection was conducted on 03/08/2023 between the hours of approximately 10:00 a.m. and 12:20 p.m. to review supplemental health and safety requirements. At the start of the inspection, there were 15 children, totaling 41 points, present with 2 assistants. The provider arrived at approximately 10:08 a.m. The Family Day Home exceeded their licensed capacity of 12 children. The caregivers were also over on points for approximately 8 minutes. The children were observed napping, playing outside and engaging in free play. Positive interactions were observed during this inspection. A complete inspection of the physical plant, caregiver and children records, fire drill log, menu, attendance, and emergency procedures was completed during this inspection. A walk-through of the home was also conducted. There were no medications on site at this time. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Information gathered during the supplemental subsidy health and safety inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-790-170-G
Description: Based on vendor interview, the vendor did not maintain a current written list of all children?s allergies.

Evidence:
1. Child #1?s record indicates a lactose allergy.
2. The vendor stated that they did not have a written list of all children?s allergies.

Plan of Correction: I will do it today.

Standard #: 8VAC20-790-180-B
Description: Based on record review, the vendor did not maintain a written hard copy of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
1. Child #4, #5, #6 and #7 were present at the family day home when the inspection began at approximately 10:00 a.m.
2. Child #4, #5, #6, and #7 were not signed in when attendance was checked at approximately 10:15 a.m.

Plan of Correction: Corrected during inspection.

Standard #: 8VAC20-790-190-B-12
Description: Based on record review, the vendor did not obtain all information for children?s records.

Evidence: The record for Child #1 indicates a lactose allergy. The record for Child #1 did not include a written allergy care plan.

Plan of Correction: I will talk to the parents today.

Standard #: 8VAC20-790-430-A-3
Description: Based on record review and vendor interview, lockdown procedures were not practiced at least annually.

Evidence: The vendor stated that they had not practiced lockdown procedures.

Plan of Correction: We will do it this year.
