Comments:

An unannounced mandated monitoring inspection was conducted today from 10:00AM to 11:05AM. 10 children (22points) were directly supervised by 2 caregivers. The physical space, attendance, menu and other required documents were reviewed. No medications were on site. Age appropriate materials were available inside. Children were observed playing outside. Standard #:	8VAC20-800-800-B
Description:	Based on review of documents and interview with the Provider, the Provider did not review and document the review of the emergency preparedness plan at least annually. Evidence: The emergency preparedness plan was last updated 2/06/2022.
Plan of Correction:	The document will be reviewed and dated.
Standard #:	8VAC20-800-830-C
Description:	Based on review of records and interview with the Provider, documentation was not maintained for emergency evacuation and shelter-in-place drills. Evidence: Provider stated the drills were completed but the documentation had not been maintained.
Plan of Correction:	Evacuation drills and shelter-in-place drills will be documented from here on out.
Standard #:	8VAC20-800-180-A
Description:	Based on record review, a subsequent tuberculosis screening for a caregiver was not completed 2 years from the completion of the previous screening. Evidence: The last TB screening for Caregiver A was completed on 3/9/2021.
Plan of Correction:	A new TB screening will be completed right away.
