Comments:

A monitoring inspection was conducted on April 20, 2023 from approximately 9:23 am until 12:00 pm. There were two children present, ages; thirteen months and 2 years of age (total of 5 points) with the provider supervising. The inspector reviewed compliance in the areas of administration, care of children, physical plant, physical equipment and environment, staffing and supervision, programming, special care and emergencies and nutrition. Two children records and the provider's record was reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent thenoncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only. Description:	Based on record review and interview, the provider did not complete a criminal history check every five years. Evidence: The record for the provider documented a criminal history dated, 03/19/2018. A repeat check was required by 03/18/2023. There was not a repeat criminal history check for the provider. During an interview with the provider, it was acknowledged that a repeat check has not been completed.
Plan of Correction:	An appointment is scheduled for 4/22/2023 at 1:10pm to complete the check.
