Comments:
A renewal inspection was initiated on 5/1/2023 and concluded on 5/1/2023. There were a total of 6 children ( 20 points) observed with 2 caregivers present. The children were engaged in the following: napping, diapering, Legos. The home, playground, 3 child records, 1 new assistant record (other household and provider records are current), evacuation drills, and emergency supplies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-800-960-E
Description: Based on observation, the bottles were not labeled with the child's full name and dated.
Evidence: There was more than 1 child in care that required a bottle. None of the bottles were dated and labeled with the child's full name.

Plan of Correction: All bottles will be dated and labeled with the child's full name.

Standard #: 8VAC20-800-120-B
Description: Based on a review of assistants records, the provider has not obtained at least 2 references.
Evidence: Assistant #1 does not have 2 references on file, only 1 was currently in the record.

Plan of Correction: The provider will obtain 1 more reference for the assistant and maintain in her record for review.

Standard #: 8VAC20-800-60-B
Description: Based on a review of children records, the provider did not obtain the medical insurance policy number for each child.
Evidence: Child #1 and #2, date of enrollment 4/17/2023 does not have documentation of the medical insurance policy number.

Plan of Correction: The provider will get the missing information from the parent at pick up tonight.

Standard #: 8VAC20-800-530-F
Description: Based on observation, there was a pillow and stuffed toy observed in a crib.
Evidence: The Inspector observed a 3 1/2 month old infant asleep in the crib. A pillow was being used to prop the infant head to the side. Also a stuffed toy which was attached to the pacifier was observed in the infants crib.

Plan of Correction: The provider immediately removed the pillow at the inspectors request. The Provider will inform the parent that stuffed animals attached to the pacifier are not allowed in the crib while the infant is sleeping.

Standard #: 8VAC20-800-680-A
Description: Based on observation, the provider did not was their hands with soap and water after each diaper change.
Evidence: The provider was observed diapering a child who had wet and soiled their diaper, once finished, she diapered another child without washing her hands before diapering the second child.

Plan of Correction: All staff will wash hands after each diaper change.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the diapering surface was not cleaned and sanitized after each use.
Evidence: The provider was observed diapering a child who wet and soiled their diaper. The provider then diapered another child without cleaning the diapering surface after the first child was diapered.

Plan of Correction: The diaper changing pad will be cleaned and sanitized after each diaper change.
