Comments:
An unannounced renewal inspection was conducted on May 4, 2023. There were 7 children (17 points) with 2 caregivers present. The home, children's records, caregiver records, household member records, attendance logs, evacuation logs, medication and authorizations, changing table menu's and the first aid kit were reviewed. The children were observed having lunch. Lunch consisted of rice, chicken, carrots, blueberries and milk. Areas of non-compliance were cited on the violation notice. Standard #: 8VAC20-800-60-B
Description: Based on a review of children's records, the provider failed to obtain all required documentation for each child enrolled.

Evidence:
Child # 4's record does not contain documentation of the addresses of the two designated persons to contact in case of an emergency if the parent's can not be reached.

Plan of Correction: I will get the addresses from parent's and put in the child's file.

Standard #: 8VAC20-800-240-A
Description: Based on observation, the provider failed to ensure that Areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition. Unsafe conditions shall include, but not be limited to, the presence of poisonous plants; tripping hazards; unstable heavy equipment, furniture, or other items that a child could pull down on himself; splintered, cracked, or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting, or breakage of any equipment; head entrapment hazards; and protruding nails, bolts, or other components that could entangle or could snag skin.

Evidence:
The paint is peeling off the wall near the stairs.

Plan of Correction: I will have this corrected.
