Technical Assistance:
The provider must send documentation to the Licensing Inspector that the background checks have been requested no later than 10 days following this notification dated May 10, 2023. Once the background check is received by the provider, the results must be forwarded to the licensing inspector.

Comments:
An unannounced monitoring inspection was conducted on May 10, 2023. There were 8 children present (22 points) with 2 caregivers supervising. The home, children's records, caregiver records, household member records, evacuation logs, attendance logs, menu's cribs, bottles and first aid kits were reviewed. The children's were observed napping and playing with floor toys. Areas on non-compliance were cited on the Violation Notice. Description: Based on a review of caregiver and household member records, the provider failed to obtain a current Criminal History Record.

Evidence:
-Caregiver #1's record does not contain documentation of a current Criminal History Record. The last Criminal History Record expired 02/16.2023.
-Household Member #1's record does not contain documentation of a current Criminal History Record. The last Criminal History Record expired 02/26/2023.

Plan of Correction: I will submit forms for finger prints and will place results in caregiver and household members files.

Standard #: 8VAC20-800-760-A-1
Description: Based on observation, the provider failed to ensure that the first aid kit contains all required supplies.

Evidence:
The first aid kit did not contain a First aid instruction manuel.

Plan of Correction: I will place first manuals in all first aid kits.

Description: Based on a review of caregiver records, the provider failed to obtain all required documentation for each caregiver.

Evidence
Caregiver #2's record does not contain documentation that two or more references as to character and reputation as well as competency were check before employment.

Plan of Correction: I will obtain references and place them in the caregiver's file.

Standard #: 8VAC20-800-180-A
Description: Based on a review of caregiver and household member records, the provider failed to obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Evidence:
-Caregiver #1's record does not contain a current tuberculosis screening or test. The last test on file expired 11/17/2022.
-Household Member #1's record does not contain a current tuberculosis screening or test. The last test on file expired 11/17/2022.

Plan of Correction: I will obtain TB screening results for caregiver and household members and place results in files.

Standard #: 8VAC20-800-210-A
Description: Based on a review of caregiver records, the provider failed to ensure that caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence:
Caregiver #1 and #2's records do not contain documentation of 16 hours of annual training for 2022-2023.

Plan of Correction: The caregiver's will complete training hours and documentation will be placed in files.
