Technical Assistance:
The provider must send documentation to the Licensing Inspector that the background checks have been requested no later than 10 days following this notification dated May 11, 2023. Once the background check is received by the provider, the results must be forwarded to the licensing inspector.

Comments:
An unannounced monitoring inspection was conducted on May 11, 2023. There were 11 children (34 points) with 3 caregivers supervising. The home, children's records, caregiver's records, household member records, evacuation records, first aid certifications, attendance logs, cribs, nap mats and first aid supplies were reviewed. The children were observed playing outside and playing inside with floor toys. Areas of non-compliance were cited on the Violation Notice. Description: Based on a review of caregiver records, the provider failed to obtain a search of central registry record check.

Evidence:
Caregiver #1's record does not contain a central registry record check. The last central registry record check expired 04/27/2023.

Plan of Correction: I will obtain and submit Central Registry record check.

Standard #: 8VAC20-770-40-D-4-a
Description: Based on a review of household member records, the provider failed to obtain a documentation of a central registry record check for persons living in the home of provider.

Evidence:
Household Member #1's records do not contain a current report of a central registry record check. The last central registry record expired 04/27/2023.

Plan of Correction: I will obtain and submit Central Registry check.

Standard #: 8VAC20-800-180-A
Description: Based on a review of household member records, the provider failed to obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Evidence:
Household Member #'1's record does not contain a current tuberculosis screening form. The last tuberculosis screen for expired on 8.29.2021.

Plan of Correction: I will have current TB records for all household members.

Standard #: 8VAC20-800-210-A
Description: Based on a review caregiver records, the provider failed to ensure that In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence:
Caregiver #3's records do not contain documentation of 16 training hours.

Plan of Correction: I will find classes for caregiver in Spanish.

Standard #: 8VAC20-800-680-B
Description: Based on observation, caregiver's failed to ensure that children's hand are washed with liquid soap and warm running water after toileting or diapering.

Evidence:
Caregiver #2 did not wash the children's hands after diaper change.

Plan of Correction: I will remind caregivers that children's hands should be washed after diaper change.
