An unannounced renewal inspection was completed on Monday, May 22, 2023 at 10:50 am to 11:41 am at the family day home. There were 3 children in care, 1 household child present ages infancy to 4 years with 1 caregivers totaling 14 points. The inspector completed observations of the child in care completing feedings, tummy time, sitting in age appropriate equipment, engaged in active play, and preparing for napping in their assigned cribs. The inspector reviewed compliance in the areas of administration, personnel, physical health of caregivers and household members, caregiver training, physical equipment and environment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 3 child record and 1 caregiver, and 2 household members records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-100-A Description: Based on review of children's record and interview, in 1 of 3 records the provider did not ensure to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: In Child#2's record (start date: 2/20/23), there was no documentation of a physical examination found for review. Interview with provider confirmed physical documentation was not available for review during the inspection.Plan of Correction: The provider contacted the parent to obtain a copy of physical to include in child's record.
