Comments:
An announced initial inspection was completed on 5/24/2023 between the hours of 1:00pm-3:10pm. There were 4 children present (13 points) with the provider and one assistant. A variety of age appropriate materials were observed in the family day home. The Inspector reviewed 4 children's records, 2 caregiver/household member records, and the Emergency Preparedness Plan submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-800-750-A
Description: Based on observation, topical skin products did not meet all requirements.

Evidence:
1. Two topical products were provided by the parent for Child #1. The products were not labeled with the child's name.

Plan of Correction: We will label the skin products.

Standard #: 8VAC20-800-760-A-1
Description: Based on observation, the first aid kit did not contain all required supplies.

Evidence:
The first aid kit did not contain triangular bandages.

Plan of Correction: We will buy triangular bandages.

Description: Based on observation, the required telephone numbrs were not posted in a visible area.

Evidence:
The following numbers were not posted in a visible area:
1. A 911 or local dial number for police, fire, and emergency medical responders;
2. The responsible person for emergency backup care
3. The regional poison control center.

Plan of Correction: We will post the required numbers.

Description: Based on record review, 1 of 4 children's records reviewed did not contain all required information.

Evidence:
1. The record for Child #2 did not contain documentation of the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached

Description: Based on record review, 4 of 4 children's records reviewed did not contain documentation of viewing proof of identity within seven business days of the child's first day of attendance..

Evidence,
1. The records for Child #1,2,3, and 4 did not contain the signature of the person viewing the proof of identity document and the date the document was viewed.

Plan of Correction: We will update the forms with the signatures and dates.

Description: Based on record review, the provider did not obtain documentation that 2 of 4 children have been adequately immunized before their first day.

Evidence:
1. The records for Child #1 and Child #2 did not contain documentation of immunizations received.

Plan of Correction: We have been in ongoing contact with the parents regarding the forms and will continue to request from the parents.

Standard #: 8VAC20-800-520-A
Description: Based on observation, cribs were not provided for 2 children under 12 months of age.

Evidence:
Child #1 (age 11 months) and Child #3 (age 8 months) were present at the family day home during the inspection. There were no cribs available for use during rest time at the family day home.

Plan of Correction: We will purchase cribs.
