Violations:
Standard #: 8VAC20-800-780-B
Description: Based on record review, the provider did not review annually with the parent the emergency contact information required to ensure the information is correct, and obtain the parent's signed acknowledgment of the review for 3 children.

Evidence:
1. The most recent documented review of the emergency contact information for Child #1 was dated 1/3/2022.
2. The most recent documented review of the emergency contact information for Child #2 was dated 11/18/2020.
3. The most recent documented review of the emergency contact information for Child #3 was dated 5/03/2019,

Plan of Correction: I will review the information with the parents.

Standard #: 8VAC20-800-900-B
Description: Based on observation, children were allowed to drink while walking and lying down.

Evidence:
1. Provider was observed giving the children milk prior to naptime. Child #1 was observed walking while drinking milk. Child #2 was observed lying on the couch while drinking.

Plan of Correction: The children don't normally walk with their milk. I will make them go back to the chair.

Standard #: 8VAC20-800-580-C
Description: Based on observation and provider interview, the provider did not provide all required daily age-appropriate activities.

Evidence:
The provider is not providing the children with opportunities for vigorous outdoor play daily. The provider stated she has not been taking the children outside during the day, as the area she typically uses, the backyard of the home, is not available due to the space being utilized for storage.

Plan of Correction: We used to go outside after breakfast and before lunch. I will get the area cleaned up so that we can use it again.
