Comments:
A monitoring inspection was conducted on June 23, 2023 from approximately 9:50 am to approximately 11:45 am. There were six children in care, ranging in ages from 2 to 8 years with one caregiver/household member supervising for a total of 9 points.

The inspector reviewed compliance in the areas of administration, care of children, physical plant, physical equipment and environment, staffing and supervision, programming, special care and emergencies and nutrition.

A total of five child records, and four caregiver/household member records were reviewed. The children were observed playing with toys during the inspection. The provider's interactions with the children were positive

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not
acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Standard #: 22.1-289.036-A
Description: Based on review of four caregiver/household member records and interview, the provider did not ensure criminal history checks and sworn disclosure statements were completed every five years.

Evidence:
1.The criminal history check for caregiver/household member #1, caregiver/household member #2, caregiver/household member #3 and caregiver/household member #4 are dated, 2/20/18, which is beyond 5 years.
2.The sworn disclosure statement for caregiver/household member #2 is dated, 6/21/2018 and the sworn disclosure statements for caregiver/household member #3 and caregiver/household member #4 are dated, 5/30/2018, which are beyond 5 years.
3.During an interview with caregiver #1, it was acknowledged that repeat checks were not documented and confirmed having no record of the repeat checks being completed.

Description: Based on review of four caregiver/household member records, the provider did not obtain a current report of Tuberculosis Screening every two years from the date of the first screening for one caregiver/household member.

Evidence:
1.The most recent documentation of a tuberculosis screening for caregiver/household member #4 was dated, 03/19/2018, exceeding the requirement to renew every two years.
2.The provider stated caregiver/household member #4 did not have a current tuberculosis screening.
