Comments:
An unannounced renewal inspection was conducted today, June 22, 2023. The inspector was on site from approximately 10:00 am until 12:00 pm. There were seven family day home children and two resident children present with two caregivers supervising for a value of 16 points. Children were observed creating patterns with blocks with one caregiver. Children also looked at books and listened to a story. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of eight children?s records and two caregiver record and two household member records were reviewed. The inspector interacted with the caregiver and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: Based on review of two caregiver?s records and interview, the provider did not obtained background checks every five years.

Evidence:

1. The record for caregiver #1 contained the results of a fingerprint national criminal record check dated 05/30/2018, exceeding five years.
2. The record for caregiver #2 contained the results of a fingerprint national criminal record check dated 05/30/2018, exceeding five years.
3. The provider confirmed a request to update the fingerprint national criminal record check
had not yet been requested.

Plan of Correction: Per the provider: I will request the updated background checks today.

Standard #: 8VAC20-800-100-A
Description: Based on review of eight children?s records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child?s attendance or within 30 days after the first day of attendance.

Evidence:

The record of child #7, enrolled 07/19/2022, did not contain documentation of a physical exceeding 30 days from enrollment.

Plan of Correction: Per the provider: I will ask the parent for a new copy.

Standard #: 8VAC20-800-270-A
Description: Based on observation and interview, the provider did not ensure firearms were stored in a locked container, compartment, or cabinet and apart from ammunition.

Evidence:

The locked case containing firearms also contained a box of ammunition. The provider confirmed that the ammunition was not stored separate from the firearms.

Plan of Correction: Pert he provider: I will separate the ammunition from the firearms and ensure both are locked.
