Technical Assistance:
Inspectors discussed the following with provider: points, cribs, appropriate/safe ways to pick up children and requirements of the law for the legal amount of children that may be in care without a license.

Comments:
An unannounced monitoring inspection was conducted on 06/30/2023 to follow up on previous areas of noncompliance with the terms of the license. There were 12 children, ages 3 months- 4 years, totaling 34 points and one caregiver present upon the inspectors? arrival. A second caregiver arrived during the inspection. A total of one caregiver and one household member records were reviewed. Two inspectors arrived for the inspection at 10:45 am and departed at 12:15 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-800-960-E
Description: REPEAT VIOLATION
Based on observation, the provider failed to ensure that bottles shall be refrigerated and labeled with the child?s full name and the date, if more than one infant is in care.

Evidence: There was one unrefrigerated, unlabeled, and undated bottle laying on the changing table. There were 3 infants in care during the inspection.

Plan of Correction: Provider will label.

Standard #: 8VAC20-800-320-B
Description: Based on observation and interview, the provider failed to ensure that child-resistant protective covers shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.

Evidence: There was an uncovered electrical outlet located in the dining room area of the home. The uncovered electrical outlet was accessible to the children in care. The caregiver stated the outlet cover was recently removed by a child in care.

Plan of Correction: Provider covered outlet during inspection.

Standard #: 8VAC20-800-520-A
Description: REPEAT VIOLATION
Based on observation, the provider failed to ensure that cribs shall be provided for children from birth through 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot, rest mat or bed.

Evidence: Child 9 (age 3 months) and Child 10 (age 4 months) were observed sleeping in car seats. Only one crib was observed during the inspection and was occupied by a child. There were no additional cribs available for Child 9 and Child 10.

Plan of Correction: Provider will obtain more cribs.

Standard #: 8VAC20-800-570-A
Description: REPEAT VIOLATION
Based on observation, record review and interviews, the provider failed to ensure that a caregiver does not exceed 16 points, determined by the point system in the standards, for each caregiver.

Evidence: There were 12 children, totaling 34 points, present during the inspection. Caregiver 1 was the only caregiver present upon the inspectors? arrival to the home at approximately 10:15 am. A total of 3 caregivers are required for 34 points. Caregiver 2 was observed arriving to the home at approximately 11:18 am.
