54.1 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
1. The most recent violation notice is to be posted in the daycare for public viewing.
2. TB tests are required for all adults, 18 years of age and over, working/living in the home.
3. If the internal thermometer of the daycare refrigerator remains inoperable a stand alone thermometer should be placed inside to determine if temperatures are maintained.
4. Car seats should not be placed on top of linens/sheets and other sleep surfaces.
5. CPR/FA courses that are online only are not accepted as certifications by the State. Certification courses must include an in-person practicum. Inspector will provide a list of approved trainers.
6. The medication policy should be updated and shared with parents to reflect that medications will not be given, as the provider is not currently MAT certified.

Comments:
An unannounced monitoring inspection was conducted at 10:35am. Observed 9 children totaling 27 points with 2 caregivers upon arrival to the home. A 3rd caregiver returned to the home. Ratios were in compliance. Children were playing with toys, putting on shoes to go outside for active play in the backyard play area, washing hands, eating lunch, being diapered and taking naps. Snack served included apples, bananas, and water. The caregivers were promptly tending to the needs of the children. The daycare areas of the home were found to be clean and well supplied with toys and equipment for the children. Lunch served: rice and beans, oranges, celery, and milk/water. The provider reported to be participating in the USDA food program. Areas of non-compliance with standards and laws reviewed were found. Standard #: 8VAC20-800-100-A
Description: Based on records reviewed, the provider failed to ensure that 5 of 9 child records included documentation of a physical exam within the first 30-days of attendance.

Evidence:

1. Child #1 - 1st day of attendance was in September 2021 and to-date there is no physical exam on-file.
2. Child #4 - Child #1 - 1st day of attendance was in June 2022 and to-date there is no physical exam on-file.
3. Child #7- 1st day of attendance was in April 2023 and to-date there is no physical exam on-file.
4. Child #8 - 1st day of attendance was in April 2023 and to-date there is no physical exam on-file.
5. Child #9 - 1st day of attendance was in August 2021 and to-date there is no physical exam on-file.

Plan of Correction: I have requested updated physical exams for the children listed which will be provided by the parents ASAP.

Standard #: 8VAC20-800-540-A
Description: Based on observations made, caregivers failed to ensure that safety straps were in use when children were sitting at the activity table for lunch. Evidence: The safety straps were not fastened for 4 of 7 children sitting in the activity table seats. The safety straps were unfastened and hanging out of the bucket seats.

Plan of Correction: Since this, I make sure to check all straps are fastened for all seats
