Comments:
A Renewal inspection was initiated and concluded on 7/26/2023. There were 4 children present with the provider. The point total was 20 The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 provider / assistant / household member records were reviewed. Children were observed playing with toys and manipulatives and eating an afternoon snack. Restroom procedures, handwashing and parent pick up were also observed. The inspection started at 4:05 pm and concluded at 5:10 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.036-B-2
Description: Based on a review of records and interview, the licensee was unable to demonstrate that each caregiver or adult household member submitted to fingerprinting for the purpose of a Criminal Record Check.
Evidence:
Four provider / assistant / household member records were reviewed.
The record for household member #4 who recently turned 18 years of age did not contain the results of a Criminal Record Check.
The provider acknowledged that she had not obtained a Criminal Record Check for household member #4.

Plan of Correction: The provider will make an appointment for household member #4 to be fingerprinted for the purpose of a criminal history check within 10 days.
Documentation of the appointment will be sent to the licensing inspector and the results of the Criminal History Check will be forwarded to the licensing inspector upon receipt.

Standard #: 8VAC20-770-40-D-4-a
Description: Based on a review of records and interview, the licensee was unable to demonstrate that each caregiver or adult household member completed, signed and dated a Sworn Statement or Affirmation form.
Evidence:
Four provider / assistant / household member records were reviewed.
The record for household member #4 who recently turned 18 years of age did not contain a signed and dated a Sworn Statement or Affirmation form.
The provider acknowledged that she had not obtained a signed and dated a Sworn Statement or Affirmation form for household member #4.

Plan of Correction: The provider will obtain a signed sworn statement or affirmation form from household member #4 and A copy will be sent to the licensing inspector within 10 days.

Standard #: 8VAC20-800-170-A
Description: Based on a review of records and interview, the provider was unable to demonstrate that each caregiver and each adult household member obtained a Tuberculosis Screening.
Evidence:
Four provider / assistant / household member records were reviewed.
The record for household member #4 who recently turned 18 years of age did not contain documentation of the results of a Tuberculosis screening.
The provider acknowledged that she had not obtained a Tuberculosis screening for household member #4.

Plan of Correction: The provider will check to see if a Tuberculosis screening was conducted at household member #4's most recent physical and, if so, obtain a copy of the documentation. If the Tuberculosis screening has not been completed, the provider will request that household member #4 obtain a screening within 2 weeks and a copy of the results will be placed in household member #4's records upon receipt.
