Comments:

An unannounced monitoring inspection was conducted on 7/27/23 from 9:15am to 10:15am with the provider and an assistant. There were three children in care, ranging in age from three- to nine-years-old, for a total of five points. Children were observed playing with alphabet blocks and magnetic blocks, doing math worksheets, saying the Pledge of Allegiance, washing hands, and eating a morning snack. Morning snack consisted of crackers, cheese, strawberries, and milk. Three child records and two caregiver records were reviewed. The emergency drill log, attendance, and menu were also reviewed. Standard #:	22.1-289.036-A
Description:	Based on review of two caregiver records and interview, the provider did not ensure that the results of a national, fingerprint-based criminal history check were obtained every five years from the date of the last check. Evidence: The fingerprints on record for Caregiver A and Caregiver B were dated 7/17/18.
Plan of Correction:	Fingerprints were completed prior to the inspection. As soon as results are received, they will be forwarded to the Licensing Inspector.
Standard #:	8VAC20-800-140-B-3
Description:	Based on review of two caregiver records, and interview, the provider did not ensure that they maintained current certification in cardiopulmonary resuscitation (CPR). Evidence: The most recent CPR certification on record for the provider was dated 4/26/21 and expired on 4/26/23.
Plan of Correction:	We have been attempting to schedule a class through one trainer, but will reach out to another to see who we can get re-trained through faster.
