Discussed the importance of correcting non-compliance and the risk ratings regarding repeated and systemic violations.

Comments:
An unannounced Monitoring inspection was conducted today. A total of 11 children (23pts) with 2 staff. All children and staff were at a nearby playground at the beginning of the inspection. The Licensing Inspection went onsite to the playground and observed programming and supervision. Staff were strategically spaced apart to ensure adequate supervision while there. The children lined up orderly and held hands to begin the walk back to the family day home. One staff was positioned in the front of the children and the assistant was in the back. The youngest children were either held or held hands with staff. Once the children returned to the family day home, they sat quietly and waited their turn to wash hands. They transitioned to the table where they were provided cold water to drink. Activiites were given to them which involved interconnecting parts, blocks and other activities to engage them into playing with each other. Sufficient number of cots onsite. No cribs are in use at this time. The home was found neat and organized. A total of 5 children's records and all staff records were reviewed. Some repeated violations found from the last inspection. Evidence:
Staff #2 was hired over 30 days ago. A Central Registry search was not obtained to date.

Plan of Correction: I will request a search and will forward the results to the Licensing Inspector within 10 days.

Description: Based on staff record review, one staff record was incomplete.
Evidence:

Based on child record review, documentation of additional immunizations was not obtained every six months for a child under the age of 2 years.
Evidence:
Child #1 is under the age of 2 years. The last documentation of immunizations was dated in August 2022.

Plan of Correction: Immunizations will be required from this parent.

Description: REPEAT:
Based on staff record review, documentation of orientation was not found for one staff.
Evidence:
Staff #2's file did not contain documentation of orientation training.

Plan of Correction: This staff will have orientation training completed by tomorrow.

Description: Based on observation of the family day home, one area of the home was not maintained in an operable condition.
Evidence:
The smoke detector located on the lower level of the family day home was not operable.

Plan of Correction: A battery will be replaced today.
