An unannounced renewal inspection was conducted on August 9, 2023 from approximately 9:50 am to 11:00 am. Provider was present with four children. Children were observed participating in various floor activities. Provider and four child records were reviewed. Violations are noted on the Violation Notice.

Standard #: 8VAC20-800-60-B Description: Based on review, provider did not ensure 2 of 4 child records have documentation of required
information.
Evidence: The following was found during review of children's records:
1. The record for child #1 (start date 6/6/2023) did not have a physical examination.
2. The record for child #4 (start date 5/3/2021) did not have parents' employment phone
numbers.Plan of Correction: Provider will get missing information from parents.
