8VAC20-800-530B: cribs when being used by infants shall have a tight-fitting bottom cover. Loose crib sheets are a suffocation hazard and increase the risk of sudden unexplained infant death.

Comments:
An unannounced renewal inspection was conducted from 9:30 - 10:30 am with the provider and her assistant. There were seven children present, ranging from 18 months to four years of age, for a total of 15 points. Children were observed playing with age-appropriate toys, singing songs, and finishing eating breakfast/morning snack which consisted of a peanut butter and jelly sandwich and milk. Two child records and three caregiver records were reviewed along with the daily attendance, emergency drill log, required posted information, menu, ratios and indoor and outdoor space. The provider and both assistants completed First Aid/CPR certification 5/26/2023 but are not certified in Medication Administration Training (MAT) so cannot administer medications to children in care.

Description: Based on review of documentation, it was determined that shelter-in-place drills were not practiced a minimum of twice a year, as required. Evidence: the last documented shelter-in-place drill was conducted 7/22/2022.

Standard #: 8VAC20-800-210-A
Description: Based on review of documentation, and an interview with the provider, it was determined that not all caregivers completed a minimum of sixteen clock hours of training annually. Evidence: The record for caregiver #1 had 5.5 hours of annual training documented. The records for caregiver #2 and caregiver #3 did not contain documentation of any annual training.

Description: Based on observation, it was determined that not all small appliances were unplugged when not in use. Evidence: a toaster oven and an air fryer were sitting on the kitchen counter plugged in. The provider stated that they were not in use at the time of the inspection.

Description: Based on observation, it was determined that the feeding table was not used according to the manufacturer's instructions when occupied by a child, with the safety strap used and securely fastened. Evidence: two children were observed sitting in the feeding table and the safety strap was not used or securely fastened.
