The Central Criminal Records Exchange was called to follow-up on the FPCs for HM #1. It was confirmed that this finding was emailed to the Provider on 03/07/23. An additional copy of this finding was sent again today (08/17/23) to the Provider.
Discussion that Child #3 is still within the grace period (30 days from their start date) for obtaining the documentation of their physical examination.

This monitoring inspection was initiated by licensing staff and concluded on 08/17/2023, between the hours of 9:30 am to 11 am. There were 3 children present (8 points), ranging from infant to preschool-age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 3 children records, 1 household record, and 1 staff record was reviewed.
Information gathered during the inspection determined that there was (1) non-compliance with an applicable code. This violation was documented on the Violation Notice issued to the home.

Based on review, 3 of 3 children files indicated that a Liability Notice Declaration form was not available.
Evidence -
1. On the date of inspection (08/17/2023), a completed and signed Liability Notice Declaration form was not available for: Child #1 (start date - 02/28/22); Child #2 (start date - 12/15/22); Child #3 (start date - 07/23/23).
2. Each form while on file with the child's name, were left blank; not signed or completed by each child's parent.Plan of Correction: Will obtain from each parent and place on file once completed and signed.
