An unannounced monitoring inspection was conducted on 08/17/2023 to follow up on previous areas of noncompliance with the terms of the license. There were 8 children, ages 4 months- 3 years, totaling 23 points and three caregivers present during the inspection. Two inspectors arrived for the inspection at 10:20 am and departed at 11:35 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 22.1-289.035-B-2 Description: REPEAT VIOLATION
Based on record review, the center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia.
Evidence: Caregiver 3 had a rehire date of 8/14/2023. Caregiver 3 stated they had a termination date of approximately 02/2021. No eligibility letter was available for review during the inspection. Caregiver 1 confirmed that no eligibility letter was available for review. Caregiver 3 was observed providing care, protection, supervision and guidance to children.Plan of Correction: Provider will send caregiver to do fingerprints on 8/17/2023 or 8/18/2023
Standard #: 8VAC20-770-60-B Description: REPEAT VIOLATION
Based on observation and interviews, the provider failed to ensure that an employee must not be employed until they have the person?s completed sworn statement or affirmation.
Evidence: Caregiver 3 had a rehire date of 8/14/2023. Caregiver 3 stated they had a termination date of approximately 02/2021. No sworn statement was available for review during the inspection. Caregiver 1 confirmed that no sworn statement was available for review. Caregiver 3 was observed providing care, protection, supervision and guidance to children.Plan of Correction: Provider will get sworn statement from caregiver.
Standard #: 8VAC20-800-170-A Description: REPEAT VIOLATION
Based on interviews, the provider failed to obtain from each caregiver, at the time of hire, a report documenting the absence of tuberculosis.
Evidence: Caregiver 3 had a rehire date of 8/14/2023. Caregiver 3 stated they had a termination date of approximately 02/2021. No TB screening was available for review during the inspection. Caregiver 1 confirmed that no TB screening was available for review. Caregiver 3 was observed providing care, protection, supervision and guidance to children.Plan of Correction: Provider will send caregiver to get TB screening.
