Technical Assistance:
Discussion that the Proof of Identification for Child #5 is under a 7 day grace period from their start date.

Discussion that Documentation of a Physical Examination for Child #4 is under a 30 day grace period from their start date.

Discussion that the some of the background checks on file are set to expire starting in November 2023 through February 2024. Please check all dates and update accordingly.

Comments:
This monitoring inspection was initiated by licensing staff and concluded on 08/25/2023, between the hours of 9:30 am to 11 am. There were 5 children present (15 points), ranging from infant to preschool-age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 5 children records, 3 household records, and 2 staff records were reviewed.

Information gathered during the inspection determined that there were (3) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Violations:
Standard #: 8VAC20-800-60-B
Description: Based on review, 2 of 5 children files did not contain all of the required information within each child's record.

Evidence -
1. On the date of inspection (08/25/2023), 1 of 2 emergency contact information was listed for Child #5.
2. An emergency medical authorization completed and signed by the child's parent was not available for Child #5.
3. A general permission for regularly scheduled trips was not available for Child #4, and Child #5.

Plan of Correction: All missing information will be obtained and placed on file.

Standard #: 8VAC20-800-70-A
Description: Based on review, 2 of 5 children files indicated that not all of the required documentation was contained within each file.

Evidence -
1. On the date of inspection (08/25/2023), a medication administration - decision to administer form was not available for review for Child #5.
2. A provisions of the emergency preparedness and response plan form was not available for: Child #4, and Child #5.
3. A Liability Insurance Declaration form was not available for: Child #4, and Child #5.

Plan of Correction: All missing documentation will be obtained and placed on file.

Standard #: 8VAC20-800-90-C
Description: Based on review, 1 of 5 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (08/25/2023), the current immunization page of file for Child #2 was dated - 02/07/2023.
2. Based on information on file, on the date of inspection, Child #2 was under two years of age.

Plan of Correction: An updated immunization page for Child #2 will be obtained and placed on file.
