Comments:
An unannounced monitoring inspection was conducted September 7, 2023 between 10:30am and 11:55am. There were 5 children present with one (1) caregiver supervising within the supervision guidelines. The children were observed playing outside with bubbles and water sprinkler, eating snack, diapering and toileting procedures, handwashing, and playing with other age-appropriate toys and materials. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas administration, physical plant, staffing and supervision, programing, evacuation drill logs, menu, first aid supplies, attendance, child and caregiver records, and emergency procedures. No medication on site. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the programs.

Violations:
Standard #: 8VAC20-800-100-A
Description: Based on record review, and interview, the provider did not obtain documentation of a physical examination prior to the child's attendance or within 30 days after the first day of attendance.
Evidence: 1. Child # 2 (DOE 8/23/22) record did not contain documentation of a physical examination.
2. The provider confirmed the record for Child #2 did not contain documentation of a physical examination.

Plan of Correction: I will tell the parent to bring the physical report to me.

Standard #: 8VAC20-800-90-C
Description: Based on record review and interview, the family day home did not obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements once every six months for children under the age of two years.
Evidence:
1. The record for Child #4 (DOB 12/2021) did not have documentation of an additional immunizations once every six months. The last documentation was on 10/3/22.
2. The provider confirmed the last documentation was on 10/3/22.

Plan of Correction: I will tell the parent to bring in an updated immunization record. I know she has a 15 month appointment schedule soon.

Standard #: 8VAC20-800-680-A
Description: Based on observation, the caregiver did not wash their hands with liquid soap and warm running water after diapering a child or assisting a child with toileting.
Evidence: The caregiver did not wash their hands after changing the diaper on child #5 and Child #4.

Plan of Correction: "I'm pretty good about washing my hands. I just felt overwhelmed. You were here, and I brought all the kids inside and they were wet and hungry. I was trying to be as fast as I could. I will be sure to wash each time going forward."
