Licensing Inspector (LI) will send a copy of the Assistant/Substitute Provider form to help Provider with documentation organization.

This monitoring inspection was initiated by licensing staff and concluded on 09/08/2023, between the hours of 9 am to 10:30 am. There were 5 children present (13 points), ranging from infant to preschool-age, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication (no meds on site today); special care and emergencies; and nutrition. A total of 5 children records, 1 adult household record, and 2 staff records were reviewed.
Information gathered during the inspection determined that there was (1) non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the home.
